GRAMS Community Connector Program

The Community Connectors Program works to support remote communities throughout midwest WA. Our goals are to work with you to ensure that you have the confidence and information required to understand, navigate, and participate in the National Disability Insurance Scheme (NDIS).

How Can We Help?

Key areas of support that we can offer include:

  • Providing information to the community about the NDIS
  • Providing support and assistance to people with disabilities and their families to complete the necessary access request forms
  • Supporting eligible participants with their NDIS planning meetings
  • Supporting NDIS participants or their families and/or carers to connect with Providers
  • Supporting NDIS participants as they connect with Support Coordinators
  • Checking in with NDIS participants and their families and /or carers
