The Integrated Chronic Disease Care (ICDC) is a program that partners with General Practitioners to provide a combination of care and multidisciplinary care for people living with chronic conditions such as diabetes, cardiovascular or respiratory conditions and required help accessing services.
The ICDC program focuses on improving your health and quality of life by providing free, individualised, holistic and coordinated services as well as support so you can better self-manage and receive the care needs of your condition.
